The right choice depends on your history. Well-controlled type 2 diabetes reversed by lifestyle — moratorium wins. A hip that has been fine for 3+ years but had a diagnosis 4 years ago — moratorium wins. Any chronic condition still being treated (bipolar, RA, MS, active cancer) — FMU or CPME, because moratorium will never bring it back into cover.
